My vision for Rejuvenate Health and Wellness is to equip my patients with the tools they need to accomplish their health and wellness goals. I see myself as your personal health coach and cheerleader.

Rejuvenate Health and Wellness uses a one-on-one approach to provide individualized care. I value the time I spend with my patients because I want them to remain healthy for as long as possible. Whether it is stabilizing a chronic disease, improving nutrition and fitness, mental health, or simply spending time talking to you about a major life change, I am here as your coach and health care support.

I have a special interest in preventive health focusing on nutrition, exercise, fitness and weight management. I appreciate the functional medicine approach because it focuses on why a disease is happening instead of simply treating the symptoms of a disease. I also love helping people who struggle with obesity. As someone who has struggled with weight gain, I understand the challenges of maintaining a healthy lifestyle while balancing different roles as a full time parent, spouse, and full time career. I also have interests and expertise in chronic disease management such as hypertension, diabetes, and cardiovascular health.

My background as a nurse is in critical care and cardiac nursing. I worked many years in some of the nation’s top hospitals including Mount Sinai Hospital and Columbia Presbyterian hospital. I have worked in the home health and retail space as a nurse and nurse practitioner. I earned my doctoral degree in nursing practice (DNP) and an MBA from Johns Hopkins University. I earned my Masters degree in Nursing from Maryville University in St Louis Missouri.

During my free time I enjoy spending time with my husband and two boys traveling or enjoying the great outdoors in Alaska. I also enjoy spending time in my kitchen trying new recipes or cooking up West African dishes with my family

Ashley

Ashley Koch

Certified Functional Nutritionist

Ashley’s work is collaborative, investigative, and comprehensive. In her practice, she works with adults, teens and children. Her in depth intake process allows her to understand your full health history, as there is no one who knows your body better than you. Ashley will help you explore the connection between your symptoms and lifestyle and guide you in building a roadmap to achieving your personal health goals. Her evidence-based laboratory testing can further identify inflammatory factors and biochemical imbalances to further highlight important areas of focus for your health. She identifies the missing factors when it comes to nutrition and lifestyle to help you achieve your desired outcomes, reverse chronic disease and symptoms, and improve longterm health. Her work with clients focuses rebuilding digestive health, blood sugar balance, addressing nutritional deficiencies, identifying inflammatory and biochemical imbalances, improving detoxification, and restoring your gut microbiome to prevent or reverse chronic disease.

Her education started in the Northern California farmers markets and in her kitchen. It continued as she utilized these tools to rebuild her personal health and worked alongside Functional Medicine providers to restore the health of her children. She completed her masters in Functional Medicine and Human Nutrition and received board certification as a Certified Nutrition Specialist. Jennifer Blackwell

Jennifer Blackwell

PA-C, CAQ-Psych
Physician Assistant

A woman whose journey through medicine, military service, and motherhood has shaped a deeply compassionate and functional approach to care. Jennifer brings over a decade of experience as a physician assistant, having started her military career as a logistics officer in the U.S. Army in 2009. She graduated from the prestigious Interservice Physician Assistant Program in 2015 and has continued serving her country with distinction in the Army Reserves.

Her strength and steadfastness are evident not only in her service but in the way she shows up for her patients with deep empathy, clinical excellence, and a fierce commitment to root-cause healing. Since 2020, she’s worked in telepsychiatry, supporting mental wellness with a lens of whole-person care. But what truly ignited her calling was her personal battle with chronic health challenges.

After years of struggling with weight cycling, non-alcoholic fatty liver disease, and being told to “just lose weight” despite normal labs and zero answers, Jennifer leaned into functional medicine. Books, podcasts, and personal research led her to discover what the traditional “sick care” model had missed-and she knew she had to bring that same empowerment and insight to her patients. Jennifer’s philosophy? “It’s simple…It’s not easy.” She reminds her patients to aim for progress, not perfection, and walks with them through real-life change, not just quick fixes.

Born and raised in Massachusetts, Jennifer is a proud wife of 15 years and mama to three energetic boys, plus one loyal dog and a curious cat. Outside the clinic, she’s likely camping, hiking, biking, running, or downhill skiing with her crew. She starts her mornings with a 10-minute meditation while the coffee brews and stays grounded through daily gratitude and a practice she calls “gladdening the mind intentionally savoring joyful moments.

And a couple of fun facts: she was once a high school football cheerleader and has a deep love for evolutionary biology. On hikes, you’ll find her teaching her boys how to examine animal bones and think about health from an ancestral lens. And yes-her Army service still surprises people.
